Our Pledge
Aspire Health & Wellness understands that your health information is personal and private. We are committed to protecting your medical information and complying with the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the HITECH Act, and all applicable federal and Louisiana state privacy laws. This Notice applies to all records of your care generated or maintained by Aspire Health & Wellness.

Contacting you with appointment reminders via phone, text, email, or mail, and sending health-related information.

Disclosures required by federal, state, or local law — including public health reporting, court orders, and law enforcement requests.
Health & Safety
Preventing or lessening a serious and imminent threat to the health or safety of a person or the public.
Your Rights
You have the following rights regarding your health information:
Inspect & Copy
Request access to your medical and billing records. We respond within 30 days.
Amend
Request corrections if you believe your information is incorrect or incomplete.
Accounting
Request a list of certain disclosures we have made of your PHI in the past six years.
Restrictions
Request limits on how we use or disclose your information for treatment, payment, or operations.
Confidential Communications
Request that we contact you in a specific way or at a specific location.
Paper Copy
Obtain a paper copy of this notice at any time from our front desk.
Breach Notification
Be notified if a breach of your unsecured PHI occurs, in accordance with law.
Our Duties
Aspire Health & Wellness is required by law to maintain the privacy of your PHI, provide this notice, abide by its terms, accommodate reasonable communication requests, and notify you in the event of a breach. We reserve the right to change this notice and will make any revised version available at our clinic and on our website.
How to File a Complaint
If you believe your privacy rights have been violated, you have the right to file a complaint. You will not be penalized or retaliated against for filing a complaint. You may contact our Privacy Officer or file directly with:
